This includes helping with functional activities, mobility exercises, and basic assessments following Trusted Assessor training. You’ll monitor patient progress, keep accurate records, and liaise with other members of the multidisciplinary team to ensure coordinated care. The role also includes prescribing and maintaining rehabilitation equipment, completing administrative tasks, and travelling to patient homes as part of your community-based duties.A full list of responsibilities can be found in the attached job description.The Ideal CandidateWe’re looking for someone with:Experience in NHS community rehab or as a Band 3 assistantKnowledge and experience of neurological rehabilitationNVQ Level 3 or equivalent in Health & Social CareStrong communication and interpersonal skillsAbility to work independently and manage their own caseloadCommitment to safeguarding, information governance, and safe practiceBasic IT skills and confidence using digital systemsA full UK driving licence and access to a vehicle for workDesirable:Trusted Assessor training or equivalent qualificationsGroup facilitation experience with Allied Health Professionals About The CompanyWe change lives by transforming health and care.
