What You'll Be Doing:
Join our team as a Social Services Assessor and play a key role in supporting adults in the community to live more independently. In this role, you will:
* Perform holistic assessments of clients within a community setting
* Identify and provide appropriate support interventions
* Recommend and arrange equipment or home adaptations to improve quality of life
* Collaborate closely with a multi-disciplinary team to ensure a seamless, person-centred care experience
* Maintain accurate and up-to-date records
Requirements & Qualifications:
To be successful in this role, you will need:
* A current and valid HCPC registration
* Proven experience in adult social care or community-based assessment
* Strong communication and interpersonal skills
* Ability to work both independently and as part of a multidisciplinary team
* A genuine commitment to improving the lives of vulnerable adults
Working Hours & Contract Duration:
This flexible locum position requires availability from Monday to Friday with some potential for hybrid working depending on the service's needs. The contract is ongoing , with a strong possibility of extension based on performance and service demand.
