This will include practicing as an autonomous clinician to support service delivery, responding to social, scientific, clinical, and ethical issues in line with the values and agreed model within the service. and patient demands. Self-Management is achieved by increasing the knowledge, skills and confidence a person has in managing their own health and care by putting in place interventions such as: peer support, self-management education and health coaching.
