At a time when we have received significant new investment to improve patient flow across Leeds, we are looking for dynamic and enthusiastic Occupational Therapists who will work collaboratively within our integrated Neighbourhood Teams and Active Recovery to provide person-centered rehabilitation to adults within their home environment.Our interventions are varied and include management of long-term conditions, falls prevention, urgent response to prevent hospital admissions, and facilitating hospital discharge using Home First principles to optimize independence. You will have the opportunity to develop wide and varied skillsets, with professional freedom to work holistically, using health coaching and self-management approaches working towards patient’s goals. We want our patients to see the right person at the right time, so you will triage and prioritise referrals, work as a team to learn from each other, drawing on each other’s strengths, and delegate or signpost to other professionals as required.You will have opportunity to make a positive difference to patients’ lives while enhancing your clinical and leadership skills by working closely with the wider multidisciplinary team and supported by clinical supervision, peer support and regular CPD. You will support others including therapy assistants and students and contribute to service improvement projects across the Trust and wider partners to develop therapy and rehabilitation services across Leeds.You will carry a caseload working autonomously to deliver and facilitate rehabilitation to enable patients to manage their own goals, maximising opportunities to live a fulfilling life.The role promotes remote working over 7 days between 8am and 6pm, weekend/unsocial hours attract enhanced pay. You will be aligned to a North, South or West hub area with an understanding that this is a city–wide service and there may be a requirement for cross cover working, where possible consideration will be given to any caring responsibilities.
