Are you a pharmacist looking for a change?  This is an exciting opportunity to join a fairly new team and provide expert support to the service and to patients.We are looking for an enthusiastic pharmacist who will embrace working within the community setting within Swindon. We are looking for a senior pharmacist (band 8a) but would also consider a development position (band 7-8a) if this is something you are looking towards.You will be a senior pharmacist supporting Hospital at Home and Urgent Community Response.  Hospital at Home provides a safe and efficient alternative to NHS bedded care providing treatment and care in a patient’s own home. This supports early discharge out of hospital and prevents avoidable admissions. You will provide a highly specialist clinical pharmacy service to virtual ward patients, optimising medicines and ensuring agreed changes in medication are implemented and sustained. This could involve remote consultations, home visits and attending multidisciplinary team meetings. You will be involved in developing and delivering safe and effective pathways of care to patients for a range of conditions.
