To provide effective nursing care for patients in their own homes and other community. To provide clinical care and treatment to those patients who require wound care management, administration and support with medications, and catheter care , as well as providing care and support to those patients with a life limiting illness and their families, that require palliative care support to remain at home during the last weeks of life. To work closely with the 24/7 community nursing teams who provide planned and unplanned care and develop strong working relationships with the GP, multidisciplinary teams and social care services in the delivery of care. This will involve liaising with PALS and complaints personnel. To actively be involved in the implementation and progressive use of information technology within the service ensuring compliance with Data Protection Act.Adhere to local safeguarding protocols and procedures seeking to safeguard vulnerable adults/ children from significant harm - emotional, physical, sexual or financial. To contribute to the development of nursing practice using informed research and further education as appropriate, enhancing professional knowledge. Required to have suitable transport available which is fit for purpose.This job involves substantial access to vulnerable adults and or children and as such an Enhanced Disclosure & Barring service check (DBS) is required.  The Ideal CandidateRN (Adult) first or second level registration and currently registered with the NMC.Systematic approaches to assessment, planning and evaluation of needs led care.Able to carry out clinical activities of a fully functional nurse and have experience in the management of wound care, catheter care, palliative care needs, diabetes, injectionsGood written and oral communication skills.Knowledge of legislation affecting professional practice.Good IT skills.Present their application and supporting information in a clear reflective manner.To be able to work flexibly and sensitively.Be able to work well in a team as well as the ability to work on your own.Excellent organisational skills.To be adaptable to a combination of roles which include visiting patients in their own homes and using a patient record database system to review physiological signs and symptoms. Willing to work in other areas of the Trust or Trust-wide as and when required to do so.Be able to drive and have access to a vehicle or other equivalent mobility.To fulfil the needs of service delivery to patients/service users and in line with current working, to be available seven days a week,About The CompanyWe change lives by transforming health and care.  Established in 2006 we are one of the UK's leading independent providers of community health and care services, working with health and care commissioners and communities to transform services with a focus on experience, efficiency and improved outcomes.
