Band 6 CHC Assessor - �40 per hour - Birmingham (Hybrid Working)
Location: Birmingham (Hybrid - must attend on-site visits)
Start Date: ASAP
Contract Length: 3 months (potential for ongoing contract)
Working Hours: Monday - Friday, 09:00 AM - 17:00 PM
Pay Rate: �40 per hour
We are seeking an experienced Band 6 CHC Assessor to join a dedicated team in Birmingham on a 3-month contract , with the potential for extension. This role offers flexible hybrid working , but the successful candidate must be available to conduct on-site patient assessments as required.
Key Responsibilities:
* Conducting comprehensive Continuing Healthcare (CHC) assessments in line with national frameworks
* Reviewing care packages and making recommendations for eligibility and funding
* Liaising with multidisciplinary teams, patients, and families to ensure the best possible care
* Ensuring compliance with NHS Continuing Healthcare guidelines and policies
* Managing caseloads efficiently and maintaining accurate documentation
Requirements:
* Registered Nurse, Social Worker, or Allied Health Professional with relevant CHC experience
* Previous experience conducting CHC assessments and DST (Decision Support Tool) reviews
* Strong understanding of NHS CHC framework and legislation
* Excellent communication, analytical, and report-writing skills
* Ability to work independently while collaborating with multidisciplinary teams
