Main ResponsibilitiesAs a valued Health Visitor, you will:
Deliver the Healthy Child Programme, assessing health needs and implementing care packages.
Use tools like the Family Health Needs Assessment to plan and evidence care.
Create individualized care plans alongside families, tailoring them to each child’s needs.
Work closely with other professionals, taking on a lead role when appropriate.
Support new staff and students, with guidance from our dedicated Practice Education team.

The Ideal CandidateYou’ll need to be:
A qualified SCPHN (Specialist Community Public Health Nurse) with current NMC registration.
Experienced in leading interventions to improve outcomes for children and families.
Passionate about driving positive change and open to sharing your ideas for innovation.
A driver with a valid UK license and access to a car for work.
