Job Opportunity: Band 6 Community Mental Health Team (CMHT) Practitioner

Location: Oldham, Greater Manchester
Position Type: Full-Time (Monday-Friday, 09:00am-17:00pm)
Contract Duration: 3-6 Month Ongoing Contract
Pay Rate: �26 per hour

An exciting opportunity has arisen for a skilled and experienced Band 6 Practitioner to join the Community Mental Health Team (CMHT) in Oldham on a full-time basis. This role offers the chance to make a meaningful impact on individuals' lives while working within the NHS framework.

Key Responsibilities
The successful candidate will be expected to deliver high-quality mental health care and support to service users in the community. Key duties include:

* Conducting comprehensive assessments of health and social care needs.
* Producing, monitoring, and evaluating collaborative care plans tailored to individual needs.
* Undertaking risk assessments and implementing effective risk management strategies.
* Delivering evidence-based therapeutic interventions.
* Providing education, advice, emotional, and psychological support to service users and their families.
* Actively promoting service user involvement in care planning and risk management to ensure a person-centered approach.

Requirements

* A relevant professional qualification in Mental Health Nursing, Social Work, or Occupational Therapy.
* Current registration with the appropriate governing body (NMC, HCPC, or equivalent).
* Experience working within a CMHT or a similar community-based mental health setting.
* Strong assessment, care planning, and risk management skills.
* Ability to provide evidence-based interventions and support.
