Our SET Keyworker service, which forms part of the NHS Long Term Plan for all Children and Young People with a diagnosis of Autism and/or a Learning Disability with the most complex needs, to have a Keyworker.

We have a fixed term, full time opportunity as a Children and Young Person (CYP) Keyworker, where you will be based out of one of our Essex hubs closest to you, with the expectation to travel to families throughout your region.

This is a Monday - Saturday service and will require dedication and flexibility, working between the hours of 8am and 8pm to put children and young people first.

Main ResponsibilitiesHCRG Care Group provide a range of children, young people and family services throughout Essex that are free at the point of delivery, which includes our SET Keyworker Service.

Your main responsibilities will include but not be limited to:


Accept, allocate and assess cases, working alongside the Case Load Manager to identify at-risk children/young people and their families from the Dynamic Support Register (DSR)
Ensures consistent allocation of cases to build rapport and maintain trusting consistent relationships with CY&P and their families
Lead and participate in the out of hours rota and providing clinical and caseload supervision.
Liaises with Health and social care, Education and Police colleagues to improve access, continuity of care and well planned and well managed transitions. ​

The Ideal Candidate
The ideal candidate for this role will have experience in either Autism, Learning Disabilities, Co-production & Trauma, although we will review all applications from individuals looking to gain exposure in these areas.
The experience we are looking for is not restricted to just a working environment, we believe that life experience such as a parent/carer of an autistic child is just as relevant as employment experience to this role.
To be successful in the role candidates are required to have excellent time management skills as well as the ability to organise a high case load.
You must be compassionate, resilient and dedicated to changing the lives of the families that HCRG Care Group strive to make a difference to.


Having access to a vehicle and driving licence is essential for this role.
