* Schedule: Monday to Friday (3 Days in, 2 days remote)

A Day in the Life of a Social Worker:
As a Band 6 Social Worker , you will play a pivotal role in supporting adults with varying complexities, including neurological conditions and other traumas, within an inpatient rehabilitation setting. Your responsibilities will include:
* Conducting comprehensive, autonomous assessments of patients' social and environmental needs.
* Developing and implementing tailored care and support plans to address identified needs.
* Providing support and intervention for safeguarding concerns and crisis situations.
* Using evidence-based practice and specialist skills to assess, plan, implement, and evaluate interventions.
* Collaborating with healthcare professionals, community teams, and external agencies to ensure seamless care coordination and discharge planning.
* Maintaining accurate and up-to-date case records in line with professional and organizational standards.
* Taking a lead role in organizing and addressing the health, social, and emotional needs of patients with neurological and other complex conditions.
* Keeping current with legislative changes, social care practices, and best practices to ensure high-quality service delivery.
Requirements and Qualifications:
* Current and valid registration with Social Work England .
* Proven experience in working with individuals affected by complex trauma, neurological conditions, or mental health challenges.
* Strong communication, advocacy, and interpersonal skills.
* Ability to work independently and collaboratively within a multidisciplinary team.
* Dedication to providing compassionate, person-centered care.
Working hours and contract duration:
This position will require you to work from Monday to Friday, but hours can be flexible. The contract is for 6 months
