The Learning Disability and Autism division in Bath and North East Somerset (B&NES) has an exciting opportunity to join our innovative and caring Locality Service. Our Complex Health Needs Service (CHNS) is a dynamic, multi-disciplinary team of specialist healthcare professionals delivering high-quality assessments, advice, therapeutic interventions, and support to adults with learning disabilities, complex health needs, and autism. We also extend our services to young people with complex needs who do not meet the criteria for LD or Autism.

We are looking for a Community Nurse for Learning Disabilities & Epilepsy to join our dedicated team of LD Nurses. In this role, you will support adults with learning disabilities and epilepsy, their families, and social care providers in managing epilepsy safely within the community. You will play a crucial part in facilitating epilepsy reviews, care plans, and contributing to the dementia pathway, along with a hospital liaison role. While this role does not involve epilepsy diagnosis, it offers a rewarding opportunity for those keen on developing their expertise in epilepsy care, with support and opportunities for continuous professional development (CPD).

Main Responsibilities
Undertake specialist LD nursing health assessments; this will involve development, implementation and evaluation of care plans for adults with Learning Disabilities and epilepsy.
Support service users, families and carers to access services and carry out advice, monitor and report outcomes.
Link with GP / Neurology to assist and support in the delivery of epilepsy management and review through the Annual Health Check programme.
Epilepsy monitoring and reviews, focussing on maintianing and updating epilepsy care plans and risk assssments.
Referrals will have a formal diagnosis. Responsibility for medical assessment / changes will be with the GP/ Epilepsy Neurology Consultant / Hospital Epilepsy Nurse Specialist.
Implement care plans which achieve better quality of life outcomes for individuals and their carers.

The Ideal Candidate
Nursing qualification recognised by NMC
Current registration with HCPC / NMC
2 years post qualification experience
Experience of learning disabilities, complex health and working in a community setting
Working knowledge of epilepsy
