Chapeltown Primary Care Network (PCN) have an exciting opportunity for a full or part time Clinical Pharmacist; working within the pharmacy team as well as the wider PCN and practice teams at Allerton and Westfield Medical Centre, St Martins Practice and Woodhouse Medical Practice.Chapeltown is a friendly and dynamic network caring for a diverse population with a wide range of complex health needs, supporting the management of long term conditions, while striving to tackle health inequalities.
